  Position Summary: This position is responsible for the overall coordination of the referral and admissions process. In addition, this position interacts closely with facility representatives, discharge planners and other agencies to facilitate the initiation of care. -
  Hours: -5 days/week - 40 hours/week

  POSITION REQUIREMENTS:
  
  • 
    
  • Be eighteen (18) years of age or older
    
  • At least one (1) year experience in a homecare, hospice and/or healthcare related field preferred
    
  • Ability to complete necessary documentation appropriately, legibly, and timely
    
  • Ability to handle stressful and delicate situations
    
  • Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
    
  • Valid driver's license and proof of insurance is required
