About the Role:

The Therapist is responsible for completing the Pre-Admission Screening Resident Review/Mental Health (PASRR/MH) process for individuals requesting admission into a nursing facility. Complete Resident Reviews for individuals admitted to Long Term Care Facilities. Complete Mandated Follow Ups with residents placed in a nursing facility. Responsible for ensuring completion of all necessary PASRR/MH paperwork and billing as required by the State. Complete LCHD and/or state-based assessments.

Location: 3010 Grand Ave, Waukegan, IL

Hours: 37.5 hours a week; Monday-Friday (8:30am-4:30pm)


Skills and Experience

  • Complete Level I and Level II Pre-Admission Screening Resident Review/Mental Health (PASRR/MH) within required timeframes.
  • Complete Resident Reviews for individuals discharged from State Operated Psychiatric Hospitals (SOPH) to a nursing facility after 30 days of admission.
  • Complete monthly Mandated Follow Ups for individuals placed in a nursing facility, to include but not limited to, record review, face to face meeting with individual, and completing necessary paperwork.
  • Complete PASRR/MH specialized assessments for applicants to a Supported Living Program.
  • Enter all required PASRR/MH information into the Unified Health System PAS data system and complete billing process as indicated.
  • Liaison with assigned Front Door Diversion Project agency to complete PASRR/MH for individuals in a psychiatric hospital.
  • Serves as a liaison between the program and referral partners, nursing facilities, psychiatric hospitals and supported living programs.
  • Provides atmosphere of customer service in assuring that clients are treated in a friendly and respectful manner.
  • Displays positive attitudes and behaviors in the accomplishment of job duties.
  • Focuses on client empowerment and evidence-based practices in providing treatment, seeking to assist clients to build on strengths in addressing treatment needs.
  • Assists in the orientation and training of staff to assist as back up for completing PASRR/MH.
  • Coordinates and completes regular review of clinical records for compliance with state and federal standards.
  • Maintains accountability standards required by LCHD/CHC and funding sources relative to quality and quantity of services provided by staff members.
  • Involved in quality improvement activities on an ongoing basis (such as implementing processes as a result of performance measurement reviews, participation in councils and teams, making suggestions to improve a process).
  • Takes a personal stake in the success of the program.
  • Organizes and participates in treatment-related clinical conferences and staffings on assigned clients.
  • Provides education and direction to clients, family members and/or significant others.
  • Take responsibility for training in and documenting clinical chart requirements to meet program and state standards.
  • Job Skill Assessment(s) may be required for this role.

Education, Qualifications, and Certification:

  • Master's degree in a related field, and
  • Successful completion of 1,000 practicum hours, or
  • One (1) year of documented clinical experience under the supervision of a QMHP, and
  • Licensed by the State of Illinois as a Licensed Professional Counselor (LPC), or as a Licensed Social Worker (LSW), or
  • Licensed by the State of Illinois as a Licensed Clinical Professional Counselor (LCPC), or as a Licensed Clinical Social Worker (LCSW)


An individual must be:

  • Licensed by the State of Illinois as a Licensed Professional Counselor (LPC), or as a Licensed Social Worker (LSW), or
  • Licensed by the State of Illinois as a Licensed Clinical Social Worker (LCSW), or Licensed Clinical Professional Counselor (LCPC).
