For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.Purpose: The Trauma Outreach Worker (TOW) position works collaboratively within the Advocate Health Care system as a representative of the Advocate Trauma Recovery Center (TRC). This position is responsible for meeting with patients who have experienced recent intentional trauma to create connections between the individual, trauma medical team and the TRC and appropriate outside community resources. The TOW is the first responders at Advocate Christ Medical Center when adolescents and adults have experienced an intentional trauma (e.g., shooting, stabling, assault), they will screen potential patients for trauma response and social needs, provide case management and provide links to community resources in order to reduce the risk of re~injury. The TOW will provide substantial patient outreach to ensure patients are connected to appropriate services, give trauma~informed psychoeducation, and offer informal emotional support to patients in the hospital and community settings. Additionally, this position serves as a community liaison and patient advocate that partners with community and violence prevention organizations to help obtain information about the crime, reduce retaliation, and provide wraparound services to the patient/family.Accountabilities:Patient Outreach and Resource Navigation Trauma and Resource Education Community Outreach and Advocacy Program and Care Team Operations
