Job Description:

As an MDS Coordinator at Trilogy Health Services, you will ensure the highest quality of care and life enrichment for our residents by focusing on personalized service in healthcare and assisted living. Youll be responsible for the accurate and timely completion of the Minimum Data Set (MDS) assessments required by regulatory frameworks, as well as establishing care plans that meet individual needs. Collaborate with healthcare staff to gather necessary patient information and health status updates. Complete accurate MDS assessments within federally mandated and internal timelines. Develop and implement personalized care plans based on MDS outcomes. Ensure compliance with all federal, state, and local regulatory requirements. Educate and assist healthcare staff about changes in regulations or processes related to the MDS. Regularly monitor and update care plans as residents’ needs evolve or as required by law. Proficient in MDS 3.0 Strong analytical and problem-solving skills Excellent organizational and time management skills Proven ability to communicate effectively with healthcare professionals and family members Knowledge of state and federal healthcare regulations Medical Insurance Dental Insurance Vision Insurance 401k Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required Certified MDS Coordinator (CMDSC) preferred
