Maintain accurate documentation of patient sessions and progress -
Collaborate with healthcare teams to ensure coordinated care -
Support discharge planning and facilitate continuity of care as
needed Qualifications: - Completion of accredited PTA program -
Completion of National Physical Therapy Exam (NPTE) - WI PTA
licensure/certification - First Aid Certifications (BLS) strongly
