Integrated Health Nurse - WCD - $5,000 SIGN ON BONUSGeneral Description

The Williams Consent Decree Program serves adult consumers, new and current, living with chronic mental illness and/or substance use issues. Members are encouraged to attend local drop-in centers. The center provides an environment for individuals to socialize, receive support from peers, and access computers. Clients may be referred by the state, receive assistance in locating housing, and be linked to Assertive Community Treatment.

Key Responsibilities for the Integrated Health Registered Nurse

* Care for high-risk mental health clients.
* Provides crisis intervention and nursing services to ambulatory mental health patients within the patient centered medical home model.
* Work involves direct patient contact and is performed in accordance with prescribed methods, techniques, and procedures to support mental health stability.

Scheduled Hours: 37.5 hours a week

* Monday-Friday (8am-4pm)

Essential Functions

* Provides health education to individuals and families utilizing Learning Needs Assessment.
* Assists the provider with diagnostic testing and or invasive procedures including obtaining lab specimens.
* Performs point of care testing and other tests as ordered.
* Prepares the patient for exam by provider and provides treatments, administers medications, and injections as prescribed
* Coordinates case management referrals/activities for patients.
* Provides aggressive outreach to hard to reach clients through home visits and appropriate treatment methods, as applicable.
* Organizes and participates in treatment-related clinical conferences and staffing's on assigned clients.
* Conducts crisis intervention/evaluation referral to persons who present themselves at the program.
* May provide direct mental health services including individual and group treatment.
* Develops and maintains a basic awareness of the Chronic Care Model Processes in order to reduce Health Disparities within our communities.
* Reports all unusual occurrences that may involve the safety and wellbeing of the patients and co-workers, directly to supervisor and/or Safety Officer.

Knowledge Skills Abilities and Education Required

Requires a Bachelor's or Associate's Degree in Nursing

* With an Associate's degree, two years of experience in psychiatric or behavioral health nursing and one year experience in a medical primary care ambulatory setting is required.
* With a Bachelor's degree, one year of experience in psychiatric or behavioral health nursing and one year experience in a medical primary care ambulatory setting is required.

Certification/Licensure(To remain in this position, the individual must possess)

* Illinois licensure as a registered nurse OR
* Verification of application upon hire and completion of Illinois licensure by reciprocity within in 6 months of the initial date of employment
* Ability to work on weekends, holidays and overtime if necessary
* Ability to maintain and keep records
