Responsibilities:
- Completes all resident program responsibilities and
expectations in an acceptable manner and consistent with
expectations outlined in residency policies, procedures, and
manuals.
- Develops and maintains competency for clinical pharmacy
practice including the evaluation of pharmacotherapy, determining
appropriate drug therapy for patients, prevention/resolution of
drug related problems, and dispensing and compounding skills
consistent with good pharmaceutical practice. Facilitates
medication-related continuity of care at admission, transfer, and
discharge.
- Reviews and accurately interprets medication orders. Evaluates
for drug-drug, drug-food, drug-lab, and drug-disease interactions.
Prepares and checks medication doses in a manner consistent with
good pharmaceutical practice, including using appropriate
compounding technique, packaging, and labeling of substance.
- Optimizes drug therapy by reviewing, evaluating, and consulting
with other healthcare providers, patients, and caregivers.
- Takes appropriate steps to prevent medication events and
reports them through appropriate channels.
- Reviews medication profiles to make recommendations regarding
the most clinically effective and cost effective drug therapy,
utilizing current clinical guidelines and evidence-based literature
as needed.
- Maintains up to date knowledge of and ensures compliance with
state and federal regulations, system policies and procedures, and
department issues and initiatives.
- Demonstrates knowledge of and provides input into establishing
and implementing drug distribution processes, including the use of
automation and technology. Assumes responsibility for departmental
operations and manages workflow to meet departmental and patient
needs in the absence of a supervisor.
- Provides work direction and guidance for technicians in
performing technical pharmacy functions and other pharmacy
caregivers. May provide orientation/training to new pharmacists and
pharmacy technicians and serves as a preceptor for interns,
students, and/or residents. Actively supports and participates in
departmental efforts, including performance improvement activities,
department committees, and/or shared governance activities.
- Must be able to demonstrate knowledge and skills necessary to
provide care appropriate to the age of the patients served. Must
demonstrate knowledge of the principles of growth and development
over the life span and possess the ability to assess data
reflective of the patient's status and interpret the appropriate
information needed to identify each patient's requirements relative
to his/her age-specific needs, and to provide the care needed as
described in the department's policies and procedures. Age-specific
information is developed further in the departmental job standards.
Licensure:
- Pharmacist, Registered Education/Experience Required:
- Doctorate Degree in Pharmacy. Typically requires 1 year of
experience in a pharmacy as a resident and has elected a second
year of training. Knowledge, Skills & Abilities Required:
- Completion of a pharmacy practice residency accredited by the
American Society of Health-System Pharmacists (ASHP). Excellent
communication skills.
- Pharmacist license issued by the state in which the team member
practices needs to be obtained within 6 months unless department
leader has determined it is not required. Physicial Requirements

- Operates all equipment necessary to perform the job. May be
exposed to disagreeable conditions due to handling of toxic and
